Hammersmith & Fulham Asthma Awareness Project

Aims: To coordinate the asthma support offered to patients and carers through integrating the support network between the local Primary Care Trust and Hospital Trust with social services, the voluntary sector and education authority across the borough.

After a delayed start the project commenced in January 2006 with a variety of awareness events held during January 2006 involving GP practices and schools and local market stalls.

Update:

  • Overall, 47 local staff were trained. All school nurses in the borough attended the training including a range of other HCP’s from primary and secondary care.
  • Hammersmith and Fulham PCT has agreed to continue to commission Asthma Study Days (2 per year) from the NRTC. Training is open to both PCT and Practice Staff in all roles.
  • Asthma UK materials have been distributed to all practices and directly to the practice nurse forum and school nursing services.
  • There is dedicated nursing support at Charing Cross and Hammersmith Hospitals ensuring that seamless care is provided to patients suffering from acute asthmatic exacerbations.
  • The number of practice nurses trained has increased from 1 to 11 during the lifetime of the project.
  • All schools have an Asthma policy.
  • 5 local GP practices were recruited for a study about the usefulness of an electronic asthma action plan. This is now available to all GP practices and promoted by the RNS service on practice visits.
